Behavioral Health

 At Divine Home Care Agency & Service, we believe behavioral health services should empower—not simply assist. Every service is built around the individual's strengths, goals, and vision for their future. We partner with clients and families to create personalized plans that foster independence, encourage community inclusion, and improve overall quality of life.

Community Living and Supports helps individuals develop the skills needed to live as independently as possible in their homes and communities. Services are individualized and may include assistance with daily living, socialization, communication, budgeting, meal preparation, community integration, and developing routines that promote long-term independence.

Community Transition Services assist eligible individuals who are moving from an institutional or restrictive setting into a home or community-based environment. This service helps ensure a smooth, safe, and successful transition while promoting long-term stability and independence.

Individual and Transitional Support provides personalized assistance to help individuals achieve greater independence, increase community participation, and successfully navigate life transitions. Services focus on building practical life skills, accessing community resources, and achieving personal goals.

Individual Placement and Support is an evidence-based supported employment service that helps individuals with behavioral health conditions obtain and maintain competitive employment. Employment specialists work alongside clients to identify career goals, search for jobs, prepare for interviews, and provide ongoing support to ensure long-term employment success.

Supported Employment assists individuals with obtaining and maintaining meaningful employment in integrated community settings. Services include career exploration, job development, workplace coaching, employer collaboration, and ongoing support designed to help individuals succeed in the workforce while increasing independence and financial stability.

Supported Living provides individualized services that enable individuals to remain safely in their own homes while receiving the supports necessary to live independently. Services may include assistance with daily routines, skill development, health and wellness monitoring, and community participation.

Respite Services provide temporary relief for family members and primary caregivers. Whether planned or provided during an unexpected situation, respite allows caregivers time to rest, attend to personal responsibilities, or recharge while ensuring their loved one continues to receive safe, compassionate care.

Peer Support Services are provided by trained individuals with lived experience in mental health or substance use recovery. Peer Support Specialists offer encouragement, mentorship, education, and guidance to help individuals build resilience, develop recovery skills, and achieve their personal goals.

Psychosocial Rehabilitation helps individuals strengthen the skills necessary for successful community living. Services focus on improving communication, coping strategies, emotional regulation, social interaction, problem-solving, independent living skills, and overall recovery.
